lost theme categories Menu on left sidebar

Hey guys, while trying to deal with my "Add to cart" issue I restored my site and in doing so I lost my left side bar categories menu and things seem a bit wonky. Please attached snippets. image 141 is the good one.

Interestingly, when I shrink the page, I do get the correct Categories hamburger menu.
Any ideas how to fix this?

Also, when I upgraded to 1.1.5 I noticed in some areas it calls it version 1.1.3

No, I had to switch to another page view to get it back. My preferred layout #1 still has this issue, so I had switch to option #3
